Step 1 of 4
Tell us who you are
It takes about 5 minutes. Start with one tap.
Step 2 of 4
Which AI tools do you see at work?
Check any you've used or seen on your unit. You don't need to know the product name.
Optional: add patient-safety work you do (sepsis, falls, med errors…)
AI tools land in the middle of work nurses already do. Check any that describe your work and roughly how many patients it has protected. Your letter uses this to show FDA what is at stake if a tool gets it wrong.
Also selected
You checked these while a different setting was selected. They stay in your letter unless you uncheck them.
Step 3 of 4
What have you seen at work?
These fit the tools you picked. Tap a question, then write a sentence or two about what you've seen at work.
No questions match that. Try a broader word like patient, alarm or monitor, or .
Something else you want FDA to know? Write it here
⚠ Leave out anything that could identify a patient: no names, dates, record numbers, or details someone could use to work out who you mean. Your comment becomes public.
For inspiration, here is how some nurses have put it. Your own sentence counts for more than any of these.
Step 4 of 4
Sign and send
Add your credentials, read your letter once, then copy it into FDA's comment form.
Sign it optional
Without a name, your letter is signed by an anonymous nurse and still counts.
Comments are posted publicly, with your name if you add one. Leave out the names of your hospital, unit and coworkers.
Only give a number you could stand behind if someone asked.
Your letter
You can edit it right here. Change any wording that doesn't sound like you.
⚠ This becomes public. Read it once for anything that could identify a patient or a colleague.
The button below confirms that you wrote this and that it doesn't identify any patient: no names, dates, record numbers or details that could single someone out.
How to send it
- Press Copy & open the FDA form. Your letter is copied and FDA's comment form opens.
- Comment: paste your letter. The box holds 5,000 characters.
- What is your comment about? Choose Health Professional - A0007.
- Attach files: skip it.
- Email address: optional. Add it and tick the box to get your tracking number by email.
- Tell us about yourself: choose An Individual. That matches how you signed your letter.
- Complete the reCAPTCHA, press Submit Comment, and screenshot your tracking number.
What you chose (edit any step)
After you submit
Ask ten nurses to do the same
One comment is a voice. Comments from nurses in their own words are what FDA can't get anywhere else. Text the link to ten nurses you know, or put the flyer up by the time clock.
Print for your unit
The flyer has a QR code that opens this app. The worksheet is for jotting notes by hand on a break, then typing them in here. Comments for this docket are submitted online.
Help us count how many nurses took part (optional)
Your name and letter are never sent.
You can also share how it went. It helps us improve the tool for other nurses.
This goes to sharonda@nursebloc.io. Your email opens so you can review it before sending.
